Important medical information

1. Inform your physician of all the medications you are taking or bring them with you to each visit. If you are visiting an eye doctor, bring your recent glasses with you.

 

2. Report your allergies to your physician.

 

3.  Be sure that when receiving a permanent medication, you always receive the same dose and in the same route. If your physician changes the dosage or your medication, be sure you are aware of the exact medication and dosage.

 

4. Tell your physician of any side effects of the medications you take or anything that is bothering you.

 

5. If your prescription is handwritten, make sure it is written in print and that it is clear.
